Transcript & Verification Request
Requests will be accepted via email ONLY and must include:
- Transcript/Verification Request Form (filled out and signed)
- Form of Payment
- Snapshot of Official Photo ID
Requirements to Release Records:
State law allows a school district to release records to the adult student directly or, upon the specific release signed by the adult student, to a third party. Therefore, New Haven Adult School requires:
- the adult student’s written signature of release (on NHAS’s Request form)
- a copy of the student’s official photo identification (e.g. driver’s license, state identification, passport)
Descriptions:
- Transcript – shows classes completed and credits earned at New Haven Adult School toward your high school diploma
- Education Verification – usually requested by a background verification company
Fees:

Processing Time:
- 5 to 7 business days
- Older transcripts and verifications may take longer
- Please note we will not be able to process requests November 22-December 10, 2024
HOW TO ORDER A TRANSCRIPT OR VERIFICATION
- Download, complete, and sign the Request Form (Click here or download below)
- Email the Request Form and a snapshot of your official photo ID to nhastranscripts@nhusd.k12.ca.us
